Instructions

Please fill out these forms as completely and accurately as possible.

If you are making multiple requests, please submit a separate request for each project. Multiple requests from the same entity must be ranked in priority order across all appropriations bills.

When asked which appropriations bill the request would best fall under, choose the single best option. We will make the determination if a project can be funded under a different bill, agnecy, or account.

For your reference, here are the primary criteria I use to consider requests:

1. Demonstrated need - does it create jobs?
2. Requested by group with proven record
3. Can't be requested by for-profit entity
4. State, local or private matching funds
5. No multi year projects (except transportation)
6. Not the first funding source
7. Is there a federal or state grant program that could fund your requests?

Requests cannot be considered if they haven't been received by Friday, March 5, 2010.
